PATIENT PRIVACY

Chime allows you to upload, submit, save, send or receive content, including photos. By using Chime you agree that you will not share any personal and confidential information, including any information that identifies a specific individual. We do not allow the unauthorized publishing of individuals’ private and confidential information, such as names, dates of surgery, credit card numbers, medical record numbers, Social Security numbers, drivers and other license numbers, phone numbers, or any other individual information, and you hereby agree never to do so on Chime.  
 
We do not want or need any individually identifiable information from you, and we are not a HIPAA business associate. As such, you cannot share protected health information (PHI) on this site; you may share only de-identified information under HIPAA.  
 
If you violate these terms of use, we will remove or restrict access to content or functionality in Chime.  
 
Chime offers editing tools, such as a blur tool, over the image to blur sensitive information. Refrain from taking screenshots of cases posted on Chime. The Chime admins will be notified of any violations of this best practice. 

If your post concerns an Exactech product-related revision surgery, adverse event, or concerns an Exactech product not meeting its therapeutic intent or otherwise not performing as expected, please report this as soon as reasonably possible to your Exactech sales representative or directly to Exactech at complaints@exac.com.
